Forms
The Forms section enables users to configure and manage data collection templates linked to specific locations. Forms support structured input for processes such as compliance, quality checks, or operational reporting.

Each form represents a data collection template (e.g., Compliance Checklist, Quality Inspection, Operational Report) and can be mapped to system data fields via worksheet logic. These mappings help ensure accurate data capture, automate workflows, and maintain consistency across locations.
The Forms section allows you to create a consistent set of data collection templates that can be assigned to locations and configured with Excel-based field mappings. These forms help ensure that structured data is captured correctly and provide visibility into compliance, quality, and operational metrics by location.
Quick Check - Before You Start
- You can edit Site Settings › Orders › Forms
- Have the latest
.xlsxtemplate and worksheet/column map agreed with compliance or QA - Confirm the Location exists before uploading so routing stays correct
Managing Forms
Add Form
- Click the Add New button at the top left.
- Select the Location from the dropdown.
- Upload the Form file (e.g., an
.xlsxExcel file). - In the Form Data section:
- Click the + icon to add rows.
- Specify details for each row: Worksheet, Column, Row, Data, and Configuration.
- Use the trashcan icon in the Actions column to remove rows.
- Click Add to save, or Save and Add to continue adding more forms.

Edit Form
- Click the pen icon next to the form you'd like to modify.
- Make the required updates to the form mappings or associated settings.
- Click Save to apply the changes.
Delete Form
Common Pitfall
Removing a form stops data capture for that location until a replacement is uploaded. Export definitions or keep a file copy before deleting.
- Single delete: Click the trashcan icon next to a form and confirm the deletion.

- Bulk delete: Use checkboxes to select multiple forms, then click the trashcan icon at the top of the table and confirm deletion.

Other Actions
- Filter – Narrow the list by Location or other criteria.
- Search – Use the search bar to locate specific forms.

Table Flexibility
Users can tailor the table layout using the following controls:

Troubleshooting
Why is submitted form data landing in the wrong field?
Worksheet, Column, and Row mappings in Form Data likely do not match the spreadsheet.
Steps to resolve:
- Open the form for edit and compare each mapping row to the Excel layout
- Re-upload the workbook if structure changed
Why doesn’t my form appear for a location?
The form may be tied to a different Location or filtered out in the list view.
Steps to resolve:
- Use Filter and Search on the Forms screen
- Add a new row for the missing location if required
Why did an upload fail silently?
File type or corrupted .xlsx files often fail validation.
